    Provides non-medical assistance to seniors and people with disabilities to maintain their independence in their home. Helps prevent unnecessary placement of elderly individuals in nursing homes.

    MSSP is a voluntary program focuses on encouraging frail, elderly residents to make healthy lifestyle choices for their social, health, and housing needs.

    Care management includes:

    • comprehensive health and psychosocial assessment
    • care planning with the client and family
    • arrangement of services
    • coordination of care with other community agencies advocacy
    • some limited payment of in home services
    • ongoing monitoring
    • annual reassessments
    • quarterly home visits and monthly contacts

    Program Delivery

    • Eligibility: Must be a current client, be 65 years of age or older homebound with health issue, such as fall risk or potential risk of being institutionalized and have Medi-Cal without share of costs
